11 NOV 65	PFC William Esposito, Jr. [CE] CPL Orin L. Allred [G] SSG Gilivaldo A. Martinez, Jr. [MD]	Vietnam	498 MED #64-13631 Aircraft shot down, crashed and burned during night mission in the central highlands. Pilots injured, enlisted crew KIA.
31 DEC 67	WO1 Thomas E. Adams [AC] WO1 William C. Cheney [CP] PFC William D. Holland [CE] C injured	Kontum Province	498 MED #66-17015 At 4600' MSL the aircraft made a left turn and crashed into trees on the hill top. Weather was a factor with strong winds at 40 knots, turbulence and low-lying clouds.
03 AUG 67	1LT Howard J. Schnabolk [P] SP5 Dwight D. Woolf [CE] SP4 Thomas L. Meidam 5 injured	Vietnam	498 MED #64-13642 Aircraft was climbing up the windward side of a mountain. As the crest of the mountain was reached, a loud noise was heard with a loss of power. An attempt was made for landing in a rice paddy. As pitch was pulled, rotor RPM was lost. The tail struck a dike, the ship nosed over and struck on the left front and burned. Power loss was caused by failure in the combustion chamber N-1 power turbine wheel.

26 OCT 70	WO1 Donald R. Cook [AC] WO1 Charles F. Smith [CP] SP4 Dennis E. Reese [CE] PFC Karl L. Reineccius [MD] OB injured	Binh Dinh Province	498 MED #67-17765 Aircraft was trying to land in extremely bad weather at night at FSB Washington to medevac a seriously ill or injured soldier. Aircraft crashed while hovering up the side of mountain to land at FB on top. Pilot lost illumination and visual contact in the fog and low clouds and caught skid in the trees, crashed and caught fire.
26 JUL 70	WO1 William A. Parker [P] 3 injured	Binh Dinh Province	498 MED #66-16632 Aircraft began a steep turn to the left, estimated to be 40' when initiated. Aircraft began to settle and consequently the M/R blades struck the water on the left hand side. Aircraft pitched forward and rolled to the right, right front cabin section struck the water and the ship inverted and began disintegrating.
